This form serves 2 purposes:
  1. For off-site small groups to make their weekly reports of off-campus activity, inventory, and supplies.
  2. For on-site small groups to request supplies.

Note that all requests made by Thursday at noon will be filled for Sunday. Items can be picked up in the Small Groups office (formerly Pastor Dixon's downstairs office, next to the front office in Building #1). You will receive notification via email of the status of your request(s), and you are responsible for picking up all requested items.

For off-site groups, you can request items to be delivered to your home, if this is more convenient for you. Please indicate on the form.
