Event Request Form

Group Leaders or Group Administrators, complete this form for small group events, except regular group meetings. 

  • Requests for trainings or other events involving current members of your small group ONLY, must be submitted 2 weeks in advance, along with a room request form. 
  • Requests for events must be submitted 60 calendar days in advance of your proposed or alternate date, whichever is earlier.
